Duo recently added universal prompt support (basically UI changes and biometrics added) and the user updated that on their end but couldn't login with below error.

Duo forcing the universal prompt upgrade. Even though customer changed that in Duo admin panel, they tried to just update the new keys in existing entry of the DUO. Which is also one of the cause of the issue.
In the OneLogin administrator portal Account settings-> Basic-> The "Enable Duo Universal Prompt" option must be enabled.
If after enabling this option you are still not able to login. Try removed the existing auth factor and re-register Duo. Then try to authenticate again.
